Sometimes life in your own home can get complicated and the support you need to stay safe and independent needs to be increased.
Country Health Connect is experienced at helping you decide when you or a loved one might need that extra level of support.
We offer more complex care through the Home Care Packages Program, which is a program funded by the Australian Government for senior Australians with needs that go beyond what the Home Support Program can provide.
Our dedicated teams across regional South Australia provide four levels of home care packages, from the basic level-one option through to the more complex level-four option.
Services are tailored to assist you to maintain your independence and continue to participate in activities that are important to you.
Services may include:
- personal care
- activities of daily living
- nutrition, meal preparation and diet
- support services, including help around the home
- clinical care and specialist advice
- respite care
- mobility and dexterity
- leisure interests and activities
Eligibility is based on your care needs as determined through a face-to-face assessment, but you must also be:
- an older person who needs coordinated services to help them stay at home
- a younger person living with disability, dementia or other care needs not met through other specialist services.

If you are to receive a home care package, the Australian Government will subsidise the cost of your care. The subsidy is paid directly to us as your service provider. We then work with you to design a care plan according to your needs and preferences, within the package budget.
