Support at Home Program

The Australian Government’s new Support at Home Program will replace the Home Care Packages program and Short‑Term Restorative Care Program from
1 November 2025. The Support at Home program offers a more comprehensive range of support, so you can continue to live well at home.

Services offered by the Support at Home Program include:

  • Support with everyday living and clinical care
  • The Assistive Technology and Home Modifications scheme for access to products, equipment and home modifications to meet your assessed needs
  • The Restorative Care Pathway, to maintain and improve your independence through allied health services
  • The End-of-Life Pathway, with extra funding to support older people to stay at home in their last 3 months of life.

Based on the assessment of your support needs, you’ll be funded to access an approved list of services. We will support you to achieve the goals set out in your care plan by accessing a mix of services within your budget. This may include:

  • Clinical care – such as nursing care or allied health services
  • Independence – such as help with showering, getting dressed, taking medications, transport or respite care
  • Everyday living – such as cleaning, gardening, shopping or meal preparation.

Your budget is determined by a classification level. The classification level reflects the complexity and duration of your support requirements.

We know that your support requirements may change from time to time – we will support you to ensure that the services you receive meet your current needs and preferences, and are keeping you safe and well. This may include seeking re-assessment if you need more or different supports.

Existing Country Health Connect client? If you are an existing client receiving services through the Home Care Packages Program or Short‑Term Restorative Care Program, your Country Health Connect provider will contact you to discuss your transition to the Support at Home Program.

New Country Health Connect client? If you wish to become a Country Health Connect client to access services through the Support at Home Program, the first step is to check your eligibility for government-funded support by contacting My Aged Care on 1800 200 422 or go online to for an assessment.

Already approved? If you have already been advised by letter that you have been approved for support through this program, please call us on 1800 944 912 to discuss the next steps.
